Class III malocclusions are among the most challenging orthodontic conditions to diagnose and treat. Their clinical appearance may result from a mandibular excess, a maxillary deficiency, dental compensation, a functional shift, or a combination of these factors. Predictable treatment begins by understanding what is truly creating the Class III relationship.
In this advanced course, Dr. Stéphane Reinhardt presents a structured and clinically relevant approach to Class III treatment with clear aligners. You will learn how to move from etiology and diagnosis to realistic treatment objectives, sound biomechanics, and effective clinical execution.
The course explores the essential differences between dental, skeletal, functional, and combined Class III malocclusions. It also examines how age, growth, facial pattern, periodontal conditions, available space, anchorage, and patient expectations influence treatment decisions.
You will discover when to retract the mandibular dentition, procline the maxillary dentition, combine movements in both arches, use Class III elastics, or consider alternative treatment approaches. Particular attention is given to staging, sequencing, anchorage preparation, attachment design, incisor position, vertical control, and the management of unwanted effects.
Growing and non growing patients are addressed separately, with an honest discussion of biological limitations, treatment compromises, orthodontic camouflage, and when surgical consultation should be considered.
Clinical cases connect the principles to real treatment decisions and demonstrate how different Class III presentations require different strategies. The goal is not to provide a single recipe. It is to help you understand the principles well enough to design an appropriate plan for each patient, regardless of the clear aligner system you use.

Learning objectives
Upon completion of this course, participants will be able to:
- Explain the genetic, environmental, skeletal, dental, and functional factors associated with Class III malocclusions.
- Differentiate between dental, skeletal, functional, and combined Class III presentations.
- Identify whether a Class III relationship results primarily from mandibular excess, maxillary deficiency, dental compensation, or a combination of factors.
- Evaluate facial characteristics, growth potential, incisor position, occlusion, periodontal conditions, and available space when assessing a Class III case.
- Establish realistic treatment objectives based on the patient’s age, diagnosis, biological limitations, and expectations.
- Determine when orthodontic camouflage may be appropriate and recognize when compromise or surgical consultation should be considered.
- Select appropriate strategies involving mandibular retraction, maxillary advancement, arch development, incisor movement, or a combination of these approaches.
- Apply biomechanical principles to staging, sequencing, anchorage, attachment design, and Class III elastic use.
- Anticipate common unwanted effects, including loss of anchorage, excessive incisor compensation, posterior interferences, and vertical changes.
- Adapt treatment planning principles to growing and non growing patients.
- Analyze clinical Class III cases and justify the selected treatment approach.
- Assess the predictability and limitations of clear aligner therapy for different Class III presentations.
